Employee Benefits

Full-time employees are eligible for FAB Benefits on the first day of the month following 30 days of service (unless otherwise specified in this document).

United Health Care Plans
PPO & HSA Insurance Plans.
FAB pays 100% of the Employee Only premiums for the PPO & HSA options.
*HRA reimbursement towards the deductible is available.

MetLife Dental Insurance Plan
Available for employee, spouse and dependant child(ren).
$50 deductible per calendar year. $5,000 benefit coverage per year.
FAB pays 100% of the Employee Only premiums

Prudential Group Term Life Insurance
FAB pays 100% towards an employee’s life insurance policy in the amount of
2 X’s the annual base salary.

Short-term Disability
Employee coverage 100% paid by First American Bank

Long-Term Disability
Employee coverage 100% paid by First American Bank

Voluntary Life Insurance Policies
Portable Term Life Insurance (employee, spouse and child coverages)
